As Chief Safety and Transformation Officer, Dr. Gandhi, MPH, CPPS is responsible for improving patient and workforce safety, and developing innovative healthcare transformation strategies. She leads the Zero Harm movement and helps healthcare organizations recognize inequity as a type of harm for both patients and the workforce. Dr. Gandhi also leads the Press Ganey Equity Partnership, a collaborative initiative dedicated to addressing healthcare disparities and the impact of racial inequities on patients and caregivers. Before joining Press Ganey, Dr. Gandhi served as Chief Clinical and Safety Officer at the Institute for Healthcare Improvement (IHI), where she led IHI programs focused on improving patient and workforce safety.

Dr. Upadhyay leads diagnostic safety efforts at Geisinger along with the organization’s Committee to Improve Clinical Diagnosis and the Division of Quality, Safety and Patient Experience. He is currently working to embed diagnostic safety in real-time patient safety/risk management infrastructure aided by his experience working full time on diagnostic safety through an exciting opportunity to translate research to meaningful implementation within a health system. Preliminary groundwork at Geisinger on diagnostic errors received a boost in 2017 through a strategic collaboration with researchers to establish a “Safer Dx Learning Lab” – a system-wide diagnostic safety surveillance program with a large portfolio of research initiatives funded by the Moore Foundation. Dr. Upadhyay led this initiative at Geisinger as the Scientist, Researcher-in-Residence and Site PI of the Lab and simultaneously assumed a key operational role in the organization. His work at Geisinger has directly informed and influenced diagnostic safety tools developed through organizations such as IHI, The Leapfrog Group and AHRQ. Dr. Upadhyay serves on the National Quality Forum’s Advancing Diagnostic Excellence Advisory Committee and is one of 12 members of national advisory panel for The Leapfrog Group’s “Recognizing Excellence in Diagnosis” - an initiative to publicly report/recognize hospitals for preventing patient harm due to diagnostic errors. He is the Co-Chair of the Practice Improvement Committee of the Society to Improve Diagnosis in Medicine. He also serves as a Faculty for Health Systems Sciences/Patient Safety at the Geisinger Commonwealth School of Medicine. Previously he was in Washington, D.C. conducting health policy research and analysis with journal Health Affairs as the Senator Lister Hill Fellow and as an health policy associate at the think-tank Urban Institute.
